• I agree to have Atelier Lash + Brow Studio to save my credit card information in their PCI compliance Square POS under my account. I  allow Atelier Lash + Brow to charge my credit card accordingly to the terms of my contract.

•  I understand that if I do not contact Atelier Lash + Brow and do not show up for my scheduled appointment that I will be charged 50% of my appointment fee.   I understand that if I do not give a 24 hour cancellation notice that will be charged a $35 fee.

•  I understand that my membership plan includes: (a.) Discounted Relash appointments that I have scheduled. (b.) Future discounts on waxing, skin care, and microblading services.

 

PAYMENT:

• I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account I am submitting.

• I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card within 7 calendar days of the original rejection date. 

• I understand that my membership will be deactivated if my account becomes more than 7 calendar days late, and that I will still be responsible for any fees incurred per my membership.

• I understand that my membership termination/cancellation request must be submitted via email to info@atelierlashbrow.com at least 7 days before my next scheduled appointment. I understand that all terms of my membership will continue to remain in effect during this 7-day interim.
